Please read this General Liability Waiver carefully. By signing below, you acknowledge and agree to the terms and conditions outlined herein in consideration for receiving services at Bellisimia Facial & Body Wellness.

1. Assumption of Risk: I understand and acknowledge that the services provided by Bellisimia Facial & Body Wellness, including but not limited to body sculpting and facials, involve inherent risks and hazards. These risks may include, but are not limited to, bodily injury, allergic reactions, and other unforeseen complications that could arise during or after the services.

2. Release of Liability: In consideration for receiving services at Bellisimia Facial & Body Wellness, I hereby release, waive, discharge, and covenant not to sue Bellisimia Facial & Body Wellness, its owners, employees, contractors, and representatives (hereinafter referred to as “Releasees”) from any and all liability, claims, demands, actions, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, while participating in or receiving services from Bellisimia Facial & Body Wellness.

3. Indemnification: I agree to indemnify, defend, and hold harmless Bellisimia Facial & Body Wellness and its Releasees from any and all costs, expenses, damages, liabilities, and attorney’s fees incurred as a result of any claims, demands, suits, or actions arising from or related to my participation in or receipt of services from Bellisimia Facial & Body Wellness.

4. Medical Conditions and Disclosure: I acknowledge that I have disclosed all relevant medical information and conditions to Bellisimia Facial & Body Wellness. I affirm that I am in good health and have no known medical or physical conditions that would prohibit me from participating in or receiving the services provided by Bellisimia Facial & Body Wellness. I understand that it is my responsibility to inform the staff of any changes to my medical condition.

5. Social Media Waiver: I understand and agree that Bellisimia Facial & Body Wellness may take photographs or videos of me during the course of receiving services. I grant Bellisimia Facial & Body Wellness, its owners, and representatives the irrevocable and unrestricted right to use, publish, and display such photographs or videos for any lawful purpose, including but not limited to advertising, promotional materials, and social media platforms, without compensation or approval rights.

6. Governing Law and Jurisdiction: This General Liability Waiver shall be governed by and construed in accordance with the laws of the State of Florida. Any legal proceedings arising out of or in connection with this waiver shall be brought in the state or federal courts located in the State of Florida.

I HAVE CAREFULLY READ AND VOLUNTARILY SIGNED THIS GENERAL LIABILITY WAIVER, AND I UNDERSTAND AND AGREE TO BE BOUND BY ITS TERMS AND CONDITIONS.
